Wellness Program

Do you have a wellness benefit? PrimeCare is now offering a complete wellness exam where you will be screened for over 60 conditions. Once the wellness visit is completed you will know what health issues need to be treated immediately and what issues need to be re-evaluated in the future. You will also receive educational materials about general health and any issues you are diagnosed with. We will also lay out a timeline for future recommended tests. The wellness exam is broken down into 2 visits. The first includes the HRA questionnaire, exam, and lab work. At the second visit, lab results will be given along with counseling about current conditions and prevention. We will bill your wellness benefit, if your insurance plan has one.

• Evidence based health risk assessment screening
• Comprehensive wellness exam
• Screening lab with blood draw (we will schedule off-site testing when necessary)
• Current disease and preventative specific education and literature
